Thank you for your recent request regarding the personal information that Aesthetic UK holds on you. Firstly, we hope you are completely satisfied with the aesthetic outcome of your treatment and look forward to seeing you again in the near future.

 

As you may be aware, the General Data Protection Regulation (GDPR) came into effect on 25th May 2018, changing the way requests for clinical records are handled. This includes the photographs taken before and after treatment, consent forms, and medical history forms.

 

These regulations also place greater responsibility on us to protect your personal data. At Aesthetic UK, your privacy and confidentiality are a top priority. We want to reassure you that your data and images will not be shared with anyone other than yourself.

 

To ensure compliance with GDPR, we require the following to process your Subject Access Request:

1. A completed request form (which we can send to you via email).

2. The signed form must be returned to us by recorded delivery.

3. A certified copy of two forms of ID must be included.
